This is why we offer Connected Care Center. We provide all of our residents and their family members free access to an online health portal. With the guardian’s and/or resident’s permission you can request access and view daily updates on your loved ones care, health, and wellness.

How to Create Your Account:

Request an invitation to create your account.

Fill out our form (and if granted authorization) one of our team members will send you a link via email to create your account.

Check your email for your registration link.

You will receive an email from Connected Care Center with a link to create your free account.

Login here and view your loved one’s real-time health information from anywhere.

You will have regular, transparent access to your loved one’s health status like blood pressure and pulse.

How do I gain access?

First, request access here. Second, you will receive an email to create your account. Thirdly, login here and get access to real-time health data on your loved one.

How do I get an account?

To create an account, contact the facility where the stay occurred and ask if they are using Connected Care Center. If they are then they will be able to register your account.

Is there a cost?
No. Connected Care Center is available to family and caregivers at no cost.
How do I log in?

You can log into Connected Care Center by clicking the log in button found at the top of the home page.

What information is available?
In accordance with the Cures Act, you can view information such as diagnosis, care plans, and lab test. For a complete list of what is available and how Connected Care Center is different from Clinical Chart click here.
Is Connected Care Center secure?
Your healthcare providers take great care to ensure your health information is kept private and secure, and so do we. Access to information is controlled through secure activation codes, personal usernames, and passwords.
The health information shown is not correct. What should I do?

The information in Connected Care Center comes directly from your healthcare providers. If you think something does not look right, please contact the health provider organization directly.

Long-Term Care Ombudsman

A long-term care ombudsman helps residents of a nursing facility and residents of an assisted living facility resolve complaints. Help provided by an ombudsman is confidential and free of charge. To speak with an ombudsman, a person may call the toll-free number 1-800-252-2412.
